Spring 2024 CAPS Intake Form
Hello, and welcome or welcome back to Bennington College Counseling & Psychological Services (or CAPS)! We're glad you're here. This form is intended for us to learn more about you and what you're looking for, and will help us connect you to the services that are the best fit for your needs. 

The information recorded here is confidential and will only be viewable in its entirety to the CAPS team. We might use generalized information for reporting data about campus therapy use to the college; in these cases, we will not disclose anything that could identify you individually (for example, we might say "150 students accessed therapy in the fall term" but we would never list your name specifically.)

Why are we collecting all this information? In order for CAPS to continue growing, envisioning and creating services that align with the diverse needs of the Bennington community, we need to better understand who seeks services and why, and report out about our work. It will also help us move towards payment models that are low-cost or free for students. This form is an attempt to start collecting that information - it's not a perfect system and we will keep working to improve it!

The form might take between 10-30 minutes to complete depending on how much detail you choose to give. If you are returning to services, it will only be a brief update. You can always decline to answer a question by skipping it or selecting the option "I choose not to answer this question."
