Ideally, this form is designed to be completed by the registrant and their supervisor together. These questions focus on setting goals and expectations for the registrant to complete the program successfully.
 
Suppose you cannot identify someone who has some authority over how you spend your time, no worries! Just complete the form by yourself with another colleague who can help hold you accountable.
 
If you are not completing this form by yourself, we suggest you set aside 30-60 minutes to meet, discuss, and complete the form with your supervisor.
 
After clicking "Submit" you will receive an email that includes your submitted responses. Additionally, a link will be provided within the same email that you can use to modify your responses if needed.
 
Halfway through the program, the supervisor will be invited to participate in a group check-in conversation with SMT staff to celebrate progress and troubleshoot any issues with meeting the goals you've identified here.
 
Note: You are not committing yourself to what you say in this form in any way from SMT's viewpoint, as this is a totally flexible program. We mainly want to make sure there has been a thoughtful internal discussion about your plans for the program.
