OCDSB  Summer School Registration Request Form for Merivale 2022
INSTRUCTIONS for Parents/Guardians and Students:

Please complete the OCDSB Summer School Registration Request Form for Merivale 2022 to request ONE summer school course. Guidance counsellors will register students online using the Ontario eLearning Consortium registration system (PRISM).

There are 3 modes of delivery for 2022 summer school programs:  

1)  OCDSB In Person/Face to Face (3 sites: Longfields Davidson Heights, Woodroffe HS and Gloucester HS  -  8:30am to 3:35 pm daily July 6th-28th)
2)  OCDSB Virtual (Synchronous and Asynchronous Learning - 8:30am to 3:35 pm daily July 6th-28th)
3)  eLearning Program (Asynchronous Learning - July 5th-August 3rd)

To confirm how your course is being delivered, please check the OCDSB Summer School website (https://ocdsb.ca/continuing_education/summer_school_programs).

Summer school start dates may vary - depending on the course format.  All programs are for the month of July.
For eLearning and Virtual courses, access to a device and internet will be necessary. Students will need to work at least 6-8 hours every weekday on their course.

Once registered, you will receive an email from the eLearning Consortium indicating that the student has been added to a waitlist.  Receipt of an email indicating that the student has been waitlisted does not mean that the course is full.

Students will remain on waitlists until there are sufficient numbers to run a course, and in many cases there will be multiple classes for courses that have many requests. When a student is accepted from the waitlist into a summer school course, they will receive an email confirming that they are registered and on a class list.

If plans change and you cannot attend summer school, please be sure to contact your guidance counselor immediately.

If you have selected "Another Course that is NOT on the list", please let us know which course.  NOTE:  If it is not on the above list, it is not offered by the OCDSB.  It may be offered by another school Board, but registration is extremely limited.
ALTERNATE COURSE CHOICE FOR 2022-2023 SCHOOL YEAR
If you selected your summer school course on Xello, Guidance will do their best to place students in their first or second alternate course choice for the 2022-2023 school year. IF an alternate is NOT required (elective, repeated course), please put "N/A" - non-applicable.

PLEASE NOTE THE FOLLOWING:
● Students should NOT schedule vacations, travel, employment, etc. during the Summer School time period. Timelines/due dates will not be adjusted to accommodate absences.
● Students taking virtual or elearning courses will need to be available for assessments/final course evaluations and should plan to be residing locally for the duration of their summer school course.
● A student may only be registered for only one Summer School course of any kind in the July Summer School Session (i.e., one In-Class Reach Ahead course or one eLearning course.)
By submitting this form, as a Parent / Guardian (or student over the age of 18), I am giving permission to Merivale staff to register my student for summer school and make the changes requested to their timetable. I understand that registration does not guarantee a spot in the summer school course, and students are encouraged to ensure that they have an alternate plan to complete required courses.
