OLC Access Form

Thanks to generous donors, strong community partners, and our hard working staff and volunteers we are able to provide access to our programs and services, from mentors to equipment, to our participants at no cost.  You will never be required to pay for anything to play with OLC, but we will ask for your time and engagement, and this "information deposit". This information is tied to grant reporting we need to do, and also provides Staff with information that can better communication and support our players and families.  

This form can be completed by either Participant or Guardian, but Guardian's need to be consulted on some of these questions. If Participant & Guardian both complete this form, the Guardian information will be used. Once this form and the OLC Waiver of Liability are completed you will be an Active Registered Member of the OLC Community. Returning participants do not need to complete this form year after year, unless you'd like to provide us with signifiant changes in Medical, Transportation, or Contact information. If you are returning participant interested in updating a smaller change, please email us at info@oaklandlacrosse.org.

OLC Player Member Benefits include:

  1. Use of our equipment to play lacrosse at any of our events. Spring team participants are also able to take home their equipment in season.
  2. Access to local opportunities to learn the sport of lacrosse, fundamentals of the game, and scaffolded game playing experiences from small sided to full field competition.
  3. Newsletters with registration information for all the upcoming programs, events and happenings at OLC and OAL
  4. If attending an OUSD Public School - support in registering for Spring OAL lacrosse teams at the middle and high school level
  5. If participating in Spring Team lacrosse with OAL - priority access to the following OLC Programs & Services: TAG, Summer Ambassadors, Youth Leadership Board, Academic/College Counseling, Wellness Workshops and powwows, and more
  6. Graduates of our program are recruited to work with OLC after high school as Coaches. 
  7. Community, unconditional love and support 
  8. FUN - lots and lots of FUN!

This is how your information will be used: 

  • Contact information helps stay in touch during the program, and secure USA Lacrosse Player Membership Benefits via OLC.  A USAL # is not required to play for OAL/CIF, however as the national governing body of lacrosse, it is required to play lacrosse in most external competitions. It also offers additional insurance benefits to the player in case of injury. 

  • Demographic information helps ensure we are meeting our equity goals and better serve all of Oakland. It is also required for almost all of our grant reports.

  • Medical and Transportation information helps us keep you safe during our events, and can also help us to remove any potential barriers to playing.  

  • Guardians listed will be the primary contact in case of emergency, and be automatically added to the player’s authorized pick up list and in program communication channels.  

  • Player info will be added to our in program communication channels only

  • Any Emergency Contact info provided below will not receive program updates, and only be contacted in case of emergency.
