ONTO Flower Submission

Please use this form to submit your ONTO Flower messages. Each message you send will be placed on a colorful paper flower. Flowers will be mailed to the recipient's address on file at the Chapter Service Center toward the end of January. Pre-orders are due by January 1, 2021. You will need to submit a new form for each flower being ordered. Pricing for 2020-21 is $2 each, 6 for $10 or 12 for $20; please be sure to send your payment payable to MNWT to COB Cathy Shuman, 9640 Goodrich Ave NE, Monticello, MN 55362.
