This order request form is to help us gauge the amount of Claudio's Pizza Kits needed. Everyone will get their desired amount of pizza kits. This is only to help us and our staff have enough supplies prepared for January 13th. Please fill out form accordingly.

Pizza Kits are $8.99 excluding tax
