NMPED Community Schools Implementation Grant Mid-Year Check-in
Please fill out this required form if you have received the NMPED Implementation Grant (Years 1-3) award for your school. This form should be filled out by the Site Based Leadership Team (which includes the CS Coordinator, Principal community partners, families, students and staff).  This form is to assist you in your progress (so be honest!). There will be no evaluation of this form; NMPED will only use this data to provide support where needed.

There are two reports required:
1. Mid-year check-in (this form) - A brief check-in of how the school is progressing on meeting key deliverables of the grant
2. End of year report (June) - MLSS Self-Assessment and CS Strategic Plan

This form is due Friday, December 15th, 2023 by 5pm.

What year of the 3-year Implementation Grant is your school in? *
CS 101 COMMUNITY MEETING: in the first two months of the beginning of the 1-year planning grant school year, the school will be required to host a “Community Schools 101” Community Meeting. This on-site meeting will focus on whole school transformation and building collaborative leadership team. The principal and at least two members of each of the following groups must be in attendance: families, students, school staff, and community partners. Any existing organization that supports a community school initiative (i.e. ABC Community School Partnership, CIS, NEA) should also be invited to this meeting. This meeting is flexible according to the community needs (e.g. booths for each video and survey questions, incentives for families, etc.) This meeting will assist the school in building the Preparatory Site Based Leadership Team.
CS COORDINATOR:  The community school has hired a full-time CS Coordinator to facilitate the Site-Based Leadership Team. The role of the CS Coordinator is in the CS strategic plan (created in the planning year). The CS Principal and Coordinator meet regularly to build a relationship of trust that will support the vision of community school implementation. *

SITE-BASED LEADERSHIP TEAM:  Families, students, teachers, principals, and community partners co-create a culture of professional learning, collective trust, and shared responsibility as they make decisions together. The site-based leadership team membership is diverse and representative of the demographic make-up of the whole-school population and the community. The SBLT meets 2x/month.

CS STRATEGIC PLAN PART 1: COLLABORATIVE LEADERSHIP  

The CS Strategic Plan requires a narrative on collaborative leadership. The following questions will be in the narrative: What is the evidence of an established site-based leadership team? Is it diverse in membership (students, families, staff and community)? How often did the SBLT meet in the Implementation Year (Year 1 or 2)? Please provide a brief summary of the SBLT. Systemic sustainability looks at the capacity and leadership of the site-based leadership team, the CS coordinator and the principal. How has the school undergone transformation in governance, power and collaborative leadership? What are the current strengths of the SBLT and areas for future improvement? Systemic sustainability ensures sustainability for the CS strategic plan and SBLT for years to come (even if there are leadership changes at the school). What are the SBLT’s specific goals that will move the school closer to whole school transformation and systemic sustainability?  Does the SBLT include membership of mutually beneficial community partners? How many partners sit as members on the SBLT (at least 50% attendance)? How do these partners participate in the implementation of the CS Strategic Plan, including collecting data and measuring outcomes for students? How can the SBLT grow and support these partnerships?

CS STRATEGIC PLAN PART 2: CS COORDINATOR 

The CS Strategic Plan requires a narrative on the CS Coordinator (CSC). The following questions will be part of that narrative: 
Please describe the role of the full-time CS Coordinator at your community school. Are the duties of the CS Coordinator in line with state statute? Does the CS Coordinator have appropriate authority for shared decision-making and responsibilities? For example, does the CS Coordinator participate in creation of the Dash plan? Does the CS Coordinator focus on the deliverables for the grant and facilitating mutually beneficial community partnerships? Does the CS Coordinator meet once a week with the CS Principal to build a relationship of trust that will support the vision of CS implementation? Please include meeting dates. Please report on the preparation of this narrative below.

CS STRATEGIC PLAN PART 4: FINANCIAL SUSTAINABILITY

The CS Strategic plan includes a narrative on financial sustainability. These questions will be included in the narrative: What is your community school sustainable funding plan and information, which can include leveraging of federal, state, local, and other spending? What is the leadership team's plan to have the CS Coordinator position and the CS strategy fully funded outside of the grant seed funding by the end of the grant period (by year 4)? How does your budget proposal for the NMPED CS grant relate to the key practices (powerful student and family engagement, collaborative leadership, shared power and voice, expanded, culturally enriched learning opportunities and integrated systems of support; supplemental: rigorous community-connected classroom instruction, culture of belonging, safety and care)? Please include information about how the funding falls into the 3 areas for the CS grant: 1) The CS Coordinator position; 2) CS-specific professional development and 3) Pipeline service providers.
