The purpose of this form is to provide Community Learning Hubs with the opportunity of selecting the teacher or substitute teacher to work during the Backpack League program. This is a MULTI-STEP PROCESS. The CLH must select the teacher/sub(s) they would like to work with this summer. In order to be paired with the site, the teacher/sub MUST apply to work the Backpack League program. Next, the teacher/sub MUST also request the Community Learning Hub as their placement. If all of these steps have been completed and the Community Learning Hub and teacher/sub select one another, the teacher/sub will be placed at the CLH.
