You can use this Form to enter a Health Report for ONE Catalan.  Please submit a new Form for each additional Catalan. Your help is greatly appreciated.

By using this Form you agree to the Catalan Sheepdog Club of Great Britain using the data you supply to inform their Health Plans and to provide reports on the health status of  UK Catalans.

All information sent to the Breed Health Co-ordinator will be treated in the strictest confidence by the Breed Health Co-ordinator.  The responses to this survey are password protected. Any personal details or identifying information relating to your Catalans (s) will not be shared publicly and will be removed from any published data.
