Each student can only take one event to the State Leadership Conference (They may also participate in a Chapter event).  If a student placed in the top 4 in two events at the District conference, they must choose which event to take to state.  If a student placed 5th-10th in one or more events, they need to fill out this form and indicate which (if any) event they will take to State. If a competitior knows they will not attend the State Conference, please have them fill this out as well.

Please indicate which event qualifiers and top 10 placers will/would like to take to the state conference. Once you have submitted this form, the student forfeits their placing for the DROPPED event and may not change their mind, as others will be notified that they've moved up the list. BE SURE your students have confirmed their choice.
