Thank you for your support of Modo Yoga, we've loved having you as a member and are sorry to see you go! Our Monthly Auto-Renew Membership requires 30 Days written notice of cancellation, so your cancellation will take effect 30 days from submission of this form. Any pre-authorized payments due to process within the next 30 days will be processed according to your membership Terms and Conditions.

Final Payment Waiver: We are happy to waive the final membership payment if you purchase a 5 class card at 10% off for a total of $90. This never expires so you can come in at your leisure.

Downgrade Membership: We actually have a "Take 5" membership, which is $89.00/month.  You would be able to use 5 classes during each billing cycle - they do not accumulate or roll over.  All other membership terms and conditions apply.

Energy Exchange Program: For just two hours a week helping out around the studio, you can take part in complimentary yoga as part of our Energy Exchange team! Please e-mail our EE Manager Bri Hawkins at energyexchange@modoyogacolumbus.com for more information.
