Butterfly and Monster Truck Custom Form
THANK YOU for participating in our Butterfly and Monster Truck Stocking! The limit for this stocking was 4 embroidered diapers and an additional 2 non-embroidered (6 total),  You MAY purchase up to 10 diapers (still a limit of 4 embroidered) if you are willing to pay actual shipping costs.  If you purchase 10 we will contact you with the shipping total due when your order is ready to ship.
After you have put in your order for your diapers, you must fill out this form by Sunday, July 18th at 11 pm CT!   IF YOU DO NOT FILL OUT YOUR FORMS BY THE TIME WE ARE READY TO PRINT CUSTOM TICKETS, WE WILL DESIGN YOUR DIAPER FOR YOU!
We have a good amount of all of the wing options, but if we max out on any wing prints, we will pull them from the form.  Some bodies may be more limited.  If something runs out, we will pull it from the form and honor any over-sales of body requests in order of INVOICE NUMBER.  TAKE YOUR TIME FILLING OUT THE FORM AS ONCE IT IS SUBMITTED, NO CHANGES MAY BE MADE!  Thank you so much and happy designing!

Embroidery disclaimer: Minor imperfections in embroidery stitching are very normal.  You may see some stitching fall outside of the border, slight gaps between border and stitching or stitches that are not completely smooth.  There may be some puckering around the embroidery where it doesn't lay flat. Breakage of the fabric is also to be expected to some extent.  It is impossible to avoid completely when embroidering stretchy laminated fabric.  To keep your embroideries looking nice and new, use FRAY CHECK at the borders where you see any little tears in the fabric (breakage) to prevent runs.  We try to pull out any with more noticeable flaws and issue discounts for 2nds, but even what passes as first quality may not always be perfect as it is just the nature of embroidery on PUL.
If you are sending in PUL to be used for wings, it must be NO SMALLER than 10" x 10" even for a single wing.  Larger is better, especially if needing a set of wings that have specific placement.

See below for thread color options.  You may suggest specific colors or give us your general vision, 'Traditional colors',  'Neon funky GN' or 'Pastel Girly with some variegated', for example.  The dragon's body and the ice cream are color blends of 2 colors, but we can do it a single color as well.  You can let us know if you want a solid color or a color blend if you'd like. Reminder that these are suggestions and although we do our best, we cannot always capture your vision.  Unfortunately, we cannot offer a remake for a missed vision or misinterpretation of your suggestion. If we don't have enough of a certain size/color, they will be awarded in order of invoice/order number.  If we over-sell, you will be contacted.  Colors marked 'old generation' are not as durable/heavy as our new gen colors.  OG prints may have slight print inconsistencies. *
