To ensure the health and safety of our staff and students, we continue to ask that students only attend class if they are healthy. Please complete the following questions to ensure your child is able to attend.
The information collected is for the sole purpose of disclosing fitness for attendance of a course and will not be used for any other purpose.
The questionnaire intends to identify NEW symptoms or worsening symptoms that may not be related to allergies, chronic or pre-existing conditions. Those with symptoms related to pre-existing, non-contagious conditions or allergies can still go to work or attend class.
