CFPA COVID-19 WAIVER 2021-2022
COVID-19 AND CONTAGIOUS DISEASE WAIVER AND RELEASE OF LIABILITY FORM

Parents MUST read and understand all guidelines before signing.



Courtney Fowler Performance Academy is offering in-person classes during the 2021-2022 year.

Due to the recommendations of the World Health Organization (WHO), the Center for Disease Control (CDC), and the Government of Newfoundland and Labrador, at CFPA we will abide by the following guidelines to help limit and slow the spread of COVID-19:
1) Maintaining a social distance of at least 6 feet;
2) Limiting class sizes;
3) Limiting contact with individuals who are sick;
4) Covering mouth and nose when sneezing or coughing;
5) Cleaning and disinfecting frequently touched objects and surfaces;
6) Avoiding touching others, including shaking hands, fist-bumping, and/or hugging;  
7) Limiting shared use items;
8) Frequently washing hands with soap and water or using hand sanitizer; and
9) Voluntarily wearing face coverings/masks when social distancing is not possible (travelling through the studio, using the washroom, etc).

Due to these recommendations and our sincere desire to limit and slow the spread of the virus, the following requirements for all of our programming is in effect. By participating, you agree to comply with the requirements below and understand and accept the consequences of your participation:

Students must bring their personal belongings in a bag, with a clear label (including items inside such as water bottles).
Snacks and food are NOT permitted in the studio unless there is a medical reason, or it is a staff member/assistant;
Students and staff must follow social distancing guidelines while inside the studio. This includes:
1) Wearing a mask while travelling through the building, wearing masks if asked, and remaining behind physical barriers,
2) Maintaining a safe social distance of a minimum of six (6) feet from staff, other students, and other individuals inside CFPA,
3) Performing personal care including but not limited to toileting and changing of clothes without the assistance of CFPA staff, or other participants
4) Self-administering medications needed during their time at CFPA to include, but not limited to placing medication in mouth, utilizing a rescue inhaler, etc. without the assistance of CFPA staff (unless a life-threatening situation)
5) Practicing positive hygiene related to hand washing and personal cleanliness including the ability to wash hands with soap and water and dry hands after washing, without the assistance of CFPA staff, and other participants
6) Understanding that some CFPA staff and participants will be wearing masks that cover the lower portion of the individual’s face.
7) Students will arrive promptly for classes/private lessons, understanding that the teacher will be waiting at the entrance for them.
8) If the student is not at the entrance after 10 minutes has passed (since the start of the lesson time), the lesson will be cancelled.
9) Students will leave the building immediately at the end of their class/lesson, to ensure the instructor has adequate time to prepare for the next class/student.

The undersigned Participant and/or legal guardian of the Participant understands that there is a global pandemic of COVID-19 and that COVID-19 may be present at any time in the local community.

The undersigned further understands that COVID-19 is a highly infectious respiratory virus that can cause serious illness and death.

The undersigned hereby acknowledges that there is an inherent risk of exposure to COVID-19 in any public place where people are present, and such risk is impossible to fully eliminate.

The undersigned hereby acknowledges their responsibility to help mitigate community spread of COVID-19 and other infectious respiratory diseases, including but not limited to influenza, measles, tuberculosis, and Severe Acute Respiratory Syndrome (SARS).

In consideration for the Participants using facilities, property, or equipment owned or controlled by CFPA, the undersigned hereby agrees to the following:
-The Participant assumes all risks related to exposure to COVID-19,
-The Participant shall not hold CFPA liable for any injury or illness,
-The Participant agrees to comply with all public health guidance and directives issued by local, provincial, or federal officials, including but not limited to guidance issued by the Center for Disease Control (CDC), the Government of Newfoundland and the Minister of Health Dr. Janice Fitzgerald, the World Health Organization (WHO), and rules established within the community of St. John’s and the Metro Region,
In the event that the Participants fail to comply with any of the terms in this agreement, CFPA and its staff shall have the sole discretion to bar Participants from entry onto the property and from taking future classes/lessons at the studio.

The undersigned further agrees that they SHALL NOT enter CFPA if:
-The Participant has been diagnosed with COVID-19 or any other infectious respiratory disease, until such time that a physician determines the Participant is no longer at risk of spreading such disease to others.
-In the previous fourteen (14) days, the Participants have experienced any symptom of COVID-19 or any other infectious respiratory disease. Such symptoms include, but are not limited to: fever, chills, cough, shortness of breath or difficulty breathing, fatigue, muscle or body aches, headache, loss of taste or smell, sore throat, congestion, runny nose, nausea, vomiting, or diarrhea.
-In the previous fourteen (14) days, the Participants have come into close contact with a person who is known to be infected with, or is suspected to be infected with, COVID-19, or any other infectious respiratory disease.
In the previous fourteen (14) days, a member of the Participant’s household has been infected with, or is suspected to have been infected with COVID-19, or any other infectious respiratory disease.
