Please use this form if you want to apply to be a model in a number of shoots I want to perform.

Please note that the modelling is on a freelance base, and not rewarded with any incentive, except a copy of the photos taken during the shoot allowing you to extend or create a portfolio.

The pictures taken during a photoshoot remain the proporty of Mel Thompson, and can be used in any form of publication by her.

The pictures however will never be sold or handed out to others with full permission rights, or transfer right.
