Light the way for the next USAHS students! Fill out this form to let us know about the high-caliber prospective student in your life. We have expanded this program to include all Physical Therapy students (DPT and DPT-FLEX) in addition to Occupational Therapy students (MOT, MOT-FLEX, MOT Hybrid Immersion, OTD and OTD-FLEX). Here’s how it works:

1. Incoming students for summer and fall 2024 and all of 2025 admission are currently eligible.
2. Referrals must be submitted prior to, or concurrent with, the student’s application using this form.
3. Referrals are not valid for students who have already submitted applications for the current term, received admissions offers or are enrolled in an OT or PT program at USAHS.
4. The award is valid for referred students who apply, are admitted and enroll within one year of receipt of the referral by the Enrollment department.
5. Spouses and children of USAHS employees are not eligible for this reward.

Students who are admitted as part of this award program and enroll in their designated program receive a $2,000 grant from USAHS. The grant will be awarded once the student's enrollment in a USAHS OT or PT program is confirmed (approximately 3 weeks after term start). Students will be responsible for any taxes associated with acceptance of the award.
