Hello 'Gansett Guardians, Students, Staff, Families and Community Members,
Nursing has partnered with Community Health Connections to provide a vaccination clinic for all people in the community wishing to receive a vaccination. We will have pediatric doses, initial doses, second doses, booster shots available for ages 5 and older, and the new bivalent booster.
Children ages 5-11 are eligible for a booster if they are greater than 2 months out of their initial vaccination series. People 12 years old and older are eligible for a booster if they are greater than 2 months from their last vaccination. All vaccine types will be available for ages 18 and older. When filling out the form, you will be able to choose which brand of vaccine you would like: Moderna or Pfizer.
Please take a minute to fill out this form if you are interested in being vaccinated at our clinic. If multiple people in your family or community want to be vaccinated, please fill out a separate form for each person so that vaccine can be reserved for them.
