CME Attendance Log
This form is only for those who intend to receive Continuing Medical Education (CME) credit for participating in the 2020 ACMT Opioid Symposium (March 12, 2020) and/or Annual Scientific Meeting (March 13-15, 2020).

If you do not intend on receiving CME credit, you do not need to fill out this form.

To receive CME credit, fill out the form and certify your attendance by identifying which sessions you actively watched. If you do not complete and submit this form, you will not receive CME. Physicians should claim only the credit commensurate with the extent of their participation in the activity.
