We know that due to the pandemic, your program delivery might have changed. We would like to offer our help with your 2019-2020 report (due July 31, 2020), if needed.

If you are able to fill out your 2019-2020 Program Outline & Outcome Measurement Evaluation (OMEs) without any issues, then please do so, using the excel sheet you filled out during the proposal phase. You can use your final report to add a narrative to the numbers you are inserting.

If you are unable to fill out the excel sheet because of the changes that occurred to your program or would simply prefer to share your numbers broken out into pre and post March 15th (cancellation of face-to-face delivery) for clarity, please fill out the form below. This form will replace the 2019-2020 Program Outline and Outcome Measurement Evaluation (OMEs).  Similarly, it will be a supplement to your final report where you will provide the narrative to your numbers.
