Please complete the information on the form below. This form is for optional kindergarten students and pre-school students who plan on attending pre-school at Fairview AND Holy Spirit. This form needs to be completed for EACH student attending preschool and optional kindergarten.

Parents/Guardians:  You will be able to change the information listed below if your circumstances change.  Changes will be made through the Carroll Community School District's Transportation Department.
