Responder MHFA - Online Course Booking Form
Please use the form below to book a place on one of the Responder MHFA Adult Online Courses. Once you have completed this booking form, please make payment for the course via a bank transfer or the Paypal link below. Your place on the course will only be confirmed upon receipt of both booking form and payment; once this has been received you will receive a confirmation email containing further details about the course. All data entered into this booking form will be kept on record until one month after the course for our own administrative purposes and will not be passed on to any third parties.

Cancellation Policy *
To cancel a booking please email jo@respondermhfa.co.uk to request a cancellation. You have the right to cancel the course and receive a full refund within 14 days of the date that you booked the course (the 'cooling off' period). This is the case unless you booked last minute and the course commences within 14 days of your booking. Cancellations outside of this cooling off period will incur a 50% cancellation fee and any cancellation requests received less than 14 days prior to the course will not be eligible for a refund.
