Join us from the comfort & privacy of your own home for a two part event via Zoom! Fill out the registration questions below to register for both the film screening of jazzy you were the art on magnolia street that will be held from 6:00-7:30 on April 15th, 2021 and the forum on Wednesday, April 21, 2021 from 6:00-7:30PM. The forum will discuss the current impact the opioid crisis is having on our immediate community with the chance to hear about local prevention and treatment efforts.

Twenty (20) of those who register will win movie night swag bags that will include a cozy blanket, snacks and SWAG from the various participating organizations. Participants selected will have the option of picking up from one of our locations (QVHD in North Haven), the Keefe Center of Hamden, OR having it dropped off to their home.
