Due to the outbreak of the novel Coronavirus, Warwick Mountains is taking extra precautions. Please be responsible and aware of your own health, read the following statements carefully and only attend events, if you are able to confirm all statements to the best of your knowledge.

I confirm that I, as well as all household members, do not currently have, nor have experienced COVID-19 symptoms (dry cough, fever, difficulty breathing, loss of smell and tase,...) within the last 14 days.

I confirm that I, as well as all household members, have not been diagnosed with COVID-19 within the past 14 days.

I confirm that I, as well as all household members, have not knowingly been exposed to anyone diagnosed with COVID-19 within the past 14 days.

I confirm that I, as well as all household members, have not travelled to any high-risk countries or regions within the past 14 days (https://www.gov.uk/guidance/travel-advice-novel-coronavirus).

I confirm that I will follow proper hygiene recommendations and wear a mask during transportation of day trips (please notify the trip organiser if you cannot wear a mask for medical reasons).

I confirm that I will notify a member of the Exec as soon as possible should I be tested positive for COVID-19 within 14 days of attending a WM event.
