*I hereby authorize the staff of the Ocean View Summer Volleyball Camp to act for me, in their best judgment, in any emergency requiring medical attention to my child. I hereby wave and release Ocean View High School, Huntington Beach Union High School District and its officers, agents, and employees, from any and all liability for any injuries, damages, or illnesses, incurred while at camp.

Along with this completed form, athletes need a current physical on file with the school.
