By registering my child for the Yates HS November Water Safety Clinic ("Clinic"), I hereby release Float2Fly, Houston Waves, HISD, its instructors, and its volunteers from liability for any injury that might occur to my child while participating in the Clinic. I agree to indemnify and hold harmless the above mentioned organizations and/or individuals, their agents and/or employees, against any and all liability for personal injury, including injuries resulting in death to my child, or damage to my property, the property of my child, or both, while my child participates in the Clinic. *

I have read the above liability waiver and agree to its terms.

I hereby grant Float2Fly and their agents the absolute right and permission to use photographic portraits, pictures, digital images or videos of my child, or in which my child may be included in whole or part, for any lawful purpose whatsoever, including but not limited to use in any Float2Fly social media account, without payment or any other consideration. *
