In an effort to make our check-in process as easy as possible for students, parents, and staff here at school, we have created this brief, multi-purpose form to address your receipt, review, and understanding of our TC 7-12 Student Handbook as well as TC Board Policy #370.00AR (TC's Co-Curricular Code). Additionally, we are including our required Chromebook Check-Out Agreement as part of this form. Please read each section of the form carefully and submit when finished.

***IT IS HIGHLY IMPORTANT THAT YOU COMPLETE AND SUBMIT ONE (1) FORM FOR EACH TC 7-12 STUDENT IN YOUR FAMILY.***

***IT IS ALSO IMPORTANT THAT EACH RESPECTIVE STUDENT(S) IS PRESENT WHEN HIS/HER FORM IS BEING COMPLETED. THEY NEED TO UNDERSTAND THE TERMS OF EACH OF THESE AGREEMENTS.***

We (TC 7-12 student AND parent) have read and understand the TC 7-12 STUDENT HANDBOOK for the 2020-2021 school year. By selecting this option, we understand that this constitutes as each of our signatures agreeing to this verification.

TC BOARD POLICY 370.00AR EX2 AGREEMENT (NON-ATHLETIC CO-CURRICULAR ACTIVITIES): All TC 7-12 students and their parents must review and acknowledge their receipt and understanding of this highly important Board Policy. NOTE: Changes to academic eligibility have been implemented for the 2020-2021 school year: shorturl.at/ejo37 *

We (TC 7-12 student AND parent) have read and understand TC BOARD POLICY #370.00AR and will abide by the terms of this code. By selecting this option, we understand that this constitutes as each of our signatures agreeing to this verification.

TC 7-12 CHROMEBOOK AGREEMENT: All TC 7-12 students and their parents must acknowedge receipt of their school-issued Chromebook for the 2020-2021 school year, as well as agree to various expectations in terms of use and care of the device. *

We (TC 7-12 student AND parent) agree to protect, keep safe, and return (in good working order) the Chromebook being issued for the 2020-2021 school year. We agree that the student will not download games or any other application not associated with meeting the educational direction given to him/her by his/her teachers. We also will promptly notify the school if the device malfunctions. Lastly, we agree to pay for the Chromebook (including charging cable) if it gets LOST or DESTROYED under our supervision - the complete replacement cost for a Chromebook, charging cable, and required device licensure is $250. By selecting this option, we understand that this constitutes as each of our signatures agreeing to these terms.
