Consent, Photo Release, and Pricing
In addition to my consent by submitting this form, and before attending camp, I will sign a hard copy form having the following wording: I, the undersigned, request that the Church of St. Luke allow my child to participate in the Church of St. Luke's Vacation Bible Camp, June 24-28, 2024.

In the event of a medical/dental emergency, I further consent to the decision made by the Church of St. Luke or any and all of it's agents relating to the provision of medical/dental assistance.

I hereby release the Church of St. Luke and all it's agents from liability arising out of or in any manner related to my child's enrollment and/or time spent at Vacation Bible Camp.
