The volunteers and employees at ARK Church have safety measures in place and will to the best of their ability help children avoid injuries, but I am aware that accidents still occasionally happen and I will not hold ARK Church or any of it's volunteers or employees responsible for accidents outside of their control.  I am also aware that ARK United Methodist Church may take photographs or make an audio or video recording of children and/or adults involved in church activities to be used for promotional and ministry purposes. I have read and agree to all of the information listed above including the publicity release and give my child permission to participate in Knights of North Castle.  If I have concerns I will reach out to Pastor Christine or another volunteer. *
