McLean HS Athletic Hall of Fame Nomination
 In order to be considered for the McLean HS Athletic Hall of Fame an application must be submitted. If you know of somebody you wish to be considered, please have them fill out this form or you can fill it out on their behalf. It is important that you provide their contact information and as much information as possible regarding their athletic accomplishments in high school, college, and professional athletics as well as any civic or professional awards they may have received.

Eligibility criteria:
- A former McLean coach, volunteer or administrator who has been retired for 2 years
- A McLean athlete that graduated at least 7 years ago
- A nominee must receive 5 out of 7 votes from the HOF Committee votes to gain election
 - A nominee's form will remain active for 5 years (if they don't get voted in).
 - After 5 years, another form will have to be submitted
