I understand the dates for the class are as follows, and no make-ups will be available.  The teachers will provide missed material for any missed classes. *

PRACTICE TEST:  The timed, proctored practice test will take place on Tuesday, 7/23, 10 am - 1 pm. I understand this is an additional fee of $55, and a detailed score report will be mailed within one week after the test is administered. *
I give permission for my student to attend the HSPT classes taught by Mrs. Ruby Amoroso and Mrs. Megan Kucia, and classes will be conducted at Divine Providence Catholic School, 2500 Mayfair Ave, Westchester, IL 60154.
*I understand that any misconduct will result in dismissal from the class. *
