Further, I understand my responsibilities pertaining to attendance, student conduct, financial obligation, and uniform care, and understand the possible consequences of failure to comply Monongalia County Schools, Morgantown High School, and Morgantown High School Band policies. *

In order to ensure that students and parents thoroughly read and understand the policies and procedures of the Morgantown High School Music Department contained within the MHS Band Handbook, each student and their parent/guardian must sign this agreement form before participating in any MHS Band Activity. As a member of the MHS Red & Blue Marching Band, I understand that I must maintain at least a 2.0 GPA and pass four major classes, I will not use, purchase, possess, or distribute alcoholic beverages, tobacco products (vapes included), controlled substances, or any form of non-perscribed or illegal drugs either in or out of the school environment, and I will conduct myself in an appropriate manner and display good citizenship when representing Morgantown High School. *
Any violation of this agreement during school time and extracurricular activities (including practice, travel, or events) will fall within the guidelines and penalties of the Monongalia County Board of Education Substance Abuse Policy. The penalties described in this policy will be enforced. If a violation occurs outside of the school environment or representation, it is the responsibility of the band member, their parent/guardian, and/or law enforcement officials to inform the school administration and the band directors of any details pertaining to the student. Following an investigation and the band member's due process, the penalties of this policy can be administered. Failure to comply with this agreement will result in the following in addition to any and all school, county, or state board of education policies: First Offense - The student will not be permitted to participate in the next scheduled event. Second Offense - The student will be suspended. Appropriate disciplinary action will be taken. Suspension could be for the remainder of the year or a determined period. Third Offense - The student will be withdrawn from the band program. *
The band department will support and cooperate with the parent/guardian in any behavioral plan they wish to implement, provided this plan does not eliminate or reduce the penalties imposed by this policy. I have read the MHS Band Handbook in its entirety, and I agree to abide by the policies and procedures contained within. Furthermore, I understand my responsibilities pertaining to attendance, student conduct, financial obligation, and uniform care. I understand the possible consequences of failure to comply with the MC BoE, MHS, and MHS Band policies. *
