Special Note: Complete this form only if you have a PK or Kinder student who has a sibling/carpool student in grades 1st-5th.
Sibling/multiple pick-up will begin on Wednesday, August 14th. However, we are already sharing this form with you prior to the first day of school so you can fill it out if your child/children qualify for this type of sibling/car pool pick-up. We will then assign your children a dismissal board number to display on a car tag provided by our campus.
If you fill out this form on or before August 4th, your child's car tag will be available for you to pick-up at our Meet the Teacher event on Friday, August 9th.
If you fill out this form and are not able to come to Meet the Teacher, your child will take home his/her car tag on the first day of school. We ask that on the first day of school for dismissal you place a piece of paper on your dashboard with your child's name and teacher's name written in large letters . This will allow our staff to ensure your child dismisses safely to you.
If this form is not filled out before Sunday, August 4th by 11:59 p.m. and you have siblings or car pool students to pick up, you will need to pick them up at each of their grade level pick-up areas. Once you complete this form, we will be able to assign you a sibling/car pool car tag.
