Commencement Participation Form Instructions:

Please take care in filling out this form as the information will be used in the ceremony's program book. Note that this form is also your ORDER for a cap & gown (you are not required to pay anything...we cover it). Caps and gowns will be distributed on campus on April 8th.

A hooder request form and program book order form will be sent out in early March. Please stay tuned for details. If you have any questions or concerns email me at rdavis@cwsl.edu.
