I the legal parent/ guardian of this participant, do hereby grant permission of the student (named on this registration form) to participate fully in said activity and all the associated activities sponsored by Highway Gospel Junior High and Youth Ministry during the Pursue Conference located at Canada Christian College in Whitby, Ontario on May 27, 2023.  I hereby forever discharge and agree to hold harmless Highway Gospel Church and the Directors thereof from any and all liability, claims or demands for personal injury, as well as damage and expenses of any nature whatsoever which may be incurred by the Parent/ Guardian (so named above) while said student is participating in the above described conference off-site.
