To participate in Stanbridge-BASSAL Athletics, please read and sign.

To: Stanbridge Academy, its officers, directors, trustees, agents, employees, administrators, representatives, volunteers, successors and assigns (“Stanbridge”):

In consideration of the Student named below (the “Student”) being permitted to participate in the after-school Athletics Program (“Athletics Program”), the undersigned, parents or legal guardians of the Student, on behalf of ourselves, our heirs, executors, administrators and assigns (“Parents”), and on behalf of the Student, and his/her heirs, executors, administrators and assigns (“Student Releasors”) (Parents and Student Releasors together the “Releasors”), hereby agree to the terms and conditions set forth below:

1. Participation: Permission is granted for the Student to participate in the Bay Area Small School Athletic League (“BASSAL”) representing Stanbridge Athletics during the 2023-2024 school year (August 23, 2023—July 26, 2024). Releasors understand and agree that participation in the Athletics Program is voluntary, that Student is not mandated by Stanbridge to participate in the Athletics Program. Releasors understand that parents of Stanbridge students may serve as drivers for the Athletics Program. Releasors hereby give permission for the Student to travel by school-owned shuttle van or with parent drivers who have completed the Parent Driver Authorization Form and participate fully in the Athletics Program.

2. Assumption of Risk: Releasors understand and acknowledge that there are risks to Student in participating in the Athletics Program. Notwithstanding these risks, Releasors hereby assume all risk of injury and harm to the Student associated with the Student’s participation in the Athletics Program and understand and agree that Stanbridge is not assuming the risk of injury or harm to the Student while he/she is participating in the Athletics Program.

Releasors understand that Athletic activities involve an inherent risk that COVID and other viruses, bacteria, and contagious illnesses may spread among BASSAL athletes and/or their family members. As preventative measures may not be sufficient to prevent the spread, releasors agree to assume the risks of exposure to communicative illnesses for their children and themselves.

3. Parent Drivers: Releasors acknowledge and agree that Stanbridge may rely upon parent drivers to transport students participating in the Athletics Program. Stanbridge requires all parent drivers to comply with its parent driver requirements, as set forth in the Parent Driver Authorization (the “Authorization”), and to execute a copy of the Authorization prior to being approved as a parent driver at the school. Releasors give their consent and permission for the Student to be transported by a parent driver while participating in the Athletics Program.

4. Expectations: Releasors understand and agree that the Student is expected to follow all rules, regulations and instructions of Stanbridge while participating in the Athletics Program and to refrain from any behavior deemed to be disruptive to the Athletics Program, harmful to the Student, or detrimental to others participating in the Athletics Program.

5. Medical Care: Releasors are aware that accidents and/or other emergencies involving the Student may occur while the Student is participating in the Athletics Program.

a. It is understood and agreed to by Releasors that, in the event of an accident or other emergency, and if they or their emergency contact designee(s) are unavailable or if it is impracticable to contact Releasors or their emergency contact designee(s), Stanbridge is hereby authorized to make such arrangements as it deems necessary for the Student to receive medical and/or hospital care, including necessary transportation. Under such circumstances, Releasors further authorize medical personnel selected by Stanbridge to undertake such care and treatment of the Student as he/she considers necessary under the circumstances.

b. Releasors understand that Stanbridge will make all reasonable efforts to contact Releasors and/or their designated emergency contact(s) as soon as possible when an emergency happens involving the Student. Releasors understand that contacting Releasors and/or their designated emergency contact(s), or attempting to communicate with said contacts, is not a prerequisite for Stanbridge to authorize or make such arrangements as he/she considers necessary for the Student to receive medical and/or hospital care, including necessary transportation, when the urgent nature of the situation necessitates such immediate action.

c. Releasors understand that reasonable minds might differ as to the particular response necessitated in a given situation. Releasors agree that Stanbridge should err on the side of seeking medical treatment most likely to protect the safety and wellbeing of the Student and, to this end, Releasors consent to any x-ray, examination, anesthetic, medical, surgical or dental diagnosis or treatment and hospital care considered necessary in the best judgment of the attending medical personnel and performed by or under the supervision of a member of the medical staff of the hospital or facility furnishing the treatment. Releasors’ authorization hereunder is given pursuant to California Family Code Section 6910. Finally, pursuant to California Health and Safety Code Section 1283, Releasors authorize the medical personnel and/or staff of the treating facility to release the Student into the custody of Stanbridge in the event that Releasors and/or their emergency contact designee(s) are unavailable to assume custody of the Student once the diagnoses and/or treatment is complete.

d. Releasors agree to assume financial responsibility for the medical services determined appropriate by Stanbridge or by the attending medical personnel.
