I am aware that families are responsible for transportation to and from 6th Grade Back-to-School camp.  Students are asked to BRING THEIR CHROMEBOOK (fully charged) and their COMBINATION LOCK (purchased on information day), any additional supplies will be provided to students during camp hours. *

Parent/Guardian, please review the following information.    Pre-Certification agreement #1:   Please indicate your agreement that you will NOT send your student to school on the day of the 6th Grade Transition Camp if they have the following symptoms: 1. If the individual is experiencing any fever (greater than 100.4 degrees), chills, muscle or body aches.  2. If the individual is experiencing; sore throat, cough, new congestion, runny nose, shortness of breath, fatigue or headache.  3. If the individual is experiencing any nausea, vomiting, diarrhea, loss of taste or smell.  4. if they recently have been in close contact with someone who has COVID-19.  We will continue to follow current health guidelines as outlined by the CDC and IDPH.   *
