I freely acknowledge that I understand and accept the following:

* A Mojo Session is a combination of Energy work, Crystal Reiki and Intuitive Consulting
* I elect to receive a Mojo Session by Jena Coray of MO MOJO, a certified Crystal Reiki Master/Teacher
* A Mojo Session is a gentle method of energy balancing and intuitive guidance, for the purpose of stress reduction, relaxation and support in self-healing.
* A Mojo Session is complementary to other healing modalities, including any medical or psychological care I may be receiving, and is not meant as a substitute for it, or to offer diagnosis or treatment.
* Jena Coray is not a medical doctor, or any other kind of medical practitioner & has not represented herself as such.
* Jena Coray will not diagnose conditions, perform medical treatment, prescribe substances, or interfere with the treatment of a licensed medical professional.
* It is recommended that I see a licensed health care professional for any physical or psychological ailment I may have.
* Jena Coray has made no promises to me as to the outcome of this Mojo Session, or the results I will achieve.
* I assume responsibility for my own health and any results from participating in a Mojo Session with Jena Coray
* All information I receive from Jena Coray is accepted with full knowledge that any action taken by myself as a result of the information I receive is my complete responsibility
* I will not hold Jena Coray liable for any perceived or diagnosed physical, emotional, mental or spiritual distress injury and/or illness that occur to me during and/or after a Mojo Session

I am of legal age and in consideration of my consent to this process, I, for myself, my heirs, executors, administrators, and assignees, do hereby release Jena Coray from all claims of damages, monies, and actions whatsoever in any manner arising from or growing out of my participation in a Mojo Session with Jena Coray.
