This survey is conducted by United Philanthropy Forum in advance of an April 2021 national strategy convening on vaccine outreach and vaccine equity. We are looking to collect as much information as possible about vaccine outreach initiatives, both in general and those prioritizing underserved communities.

In addition to providing information about particular initiatives (local, state, or national), you will have the opportunity to nominate an initiative to highlight at our national funder convening.
