This form is used by students enrolled in COEHS courses that utilize Watermark™ Student Learning and Licensure who are not seeking teacher certification. Watermark™ Student Learning and Licensure is required by COEHS in order to maintain our quality programs and provide data that is used for accreditation. Watermark™ also enables teacher candidates to complete course assignments, build electronic portfolios, and showcase educational growth.

Students are not eligible to "opt-out" of purchasing a Watermark™ subscription if they are seeking teacher certification. This form is only for students not seeking teacher certification.

I have chosen not to purchase a Watermark™ Student Learning and Licensure account because I do not intend to pursue a certification program. I do understand that I must still complete all clinical hours and assignments as a required part of course activities. However, I acknowledge that these clinical hours will not be counted toward the 200 hours required for teacher certification. If I decide to change my major and seek certification at the undergraduate or graduate level, I know I will need to take EDU 450 and work with the Director of Teacher Education Services to repeat any required clinical hours. By submitting this form, I understand these stipulations. Furthermore, I confirm I have met face-to-face with my course instructor to discuss my decision. *
