Your pet has been scheduled for a 1.5 hour initial consultation with our DERMATOLOGY department. Please complete this form 24 hours prior to your appointment. Please be as thorough as possible, as records are not always available for review prior to appointment. If you are not able to complete this form online, please call 206-204-3366 so that we can assist you in filling out this form. Press 6, then 2 and 2 to be connected with a dermatology technician. If we are unavailable, alternatively you can also press 5, then 1 to speak to a front desk staff member. If you are not able to complete the form the day prior to the appointment, this will result in significantly longer wait times and rescheduling may be necessary.
