Thank you for filling out this private singing lessons request.  Once you have submitted the request you will receive offers for a set singing time.  You will not be invoiced for Term 1 until after you have accepted your singing lessons and the day/time has been finalised.

In 2023 the following Singing lesson times are currently available...
Mondays (is booked out before 6:30pm but has availability from 6.30pm - 8:30pm), 
Wednesday has  times available from 4pm - 5pm and  5:30pm - 6:pm and 7pm - 8:30pm  
Thursdays  has times available 7:30pm - 8:30 
Saturdays from 10:0am - 10:30  and 11am - 11:30 & 11:30- 12noon.  
Please list your first 3 preferences of singing day and time.  You must list 3 preferences to receive a spot.  More days/times will be opened up when classes book up. Classes will not run on Tuesdays.

I give permission for this student to be photographed and filmed by Academy of Dance Victoria. I understand that images may be used for the purposes of advertising and promotion on media platforms including social media, website, programs and other forms of advertising.

I give permission for this student to be  filmed on stage by a professional video company hired by Academy of Dance Victoria for theatre performances for the purpose of families to purchase a copy to remember the performances by.

Please agree to the terms of enrolment. 

* Fees are payable per term in advance and are non-refundable. I understand that I am responsible for fees and those fees are to be paid by the due date listed on the invoice. I understand there are conditions regarding unpaid fees as listed in the Academy of Dance Victoria 2023 booklet and that my child cannot participate in classes if fees are not paid on time.

* There will be no refunds for missed classes. If a student is sick a makeup class can be taken in the same age group in available classes. Classes cannot be made up when social activities clash.

* Concert production fee & costume hire fees will apply in Terms 2 & 3 & 4 for productions and are non-refundable.

* In the event of an emergency – illness or accident, where I cannot be contacted, I give the staff of Academy of Dance Victoria permission to refer my child to a Doctor, Ambulance or Hospital. This will be at the Parent/guardians expense.

* Participation in any Academy of Dance Victoria activity and use of any recreational facilities involves a risk of accidental injury despite all safety precautions. Having been informed of the activities conducted by Academy of Dance Victoria, I/we, as an individual or as a parent or guardian of the participants named herein, assume all risks and hazards incidental to the activities, and release from responsibility and agree to indemnify and hold harmless Academy of Dance Victoria, its officers, directors, independent contractors, volunteers and all employees for any illness or injury to me or my children or family members occurring during his/her/our participation in any activity or use of any recreational facility at or conducted by Academy of Dance Victoria.

* All material both music and choreography are the property of Academy of Dance Victoria. Under no circumstances are the dances to be copied and used in outside activities, including school based activities and competitions (permission must be sought from the Principal to do so).

* There is no refund for fees paid in the case of lockdown. All classes will convert to an online format.

* In the case of Covid 19 all care is taken to reduce the spread of the virus and all government guidelines are followed and subject to change to stay in line with government policies. We reserve the right to restrict the number of visitors within the studio and will not be held responsible should anyone visiting the studio contract the virus.
