Please complete this form if you would like a to loan Chromebook for your child to use at home during the Coronavirus school closure period.

You will need to complete one form per child.

Please note, we have a limited number of devices available to loan, therefore priority will given to students in year 10 and those students defined as vulnerable by the government’s current guidance. Applications will be reviewed weekly and you will be notified whether your application has been successful or not.

By loaning a device, you will be agreeing to take full responsibility for all equipment issued to you by Orchard Mead Academy. We will require you collect the device with your child and to sign our “Loan Agreement” before you can take a loan device home, a copy of this agreement is available on our website.
