IMPORTANT:
** Be sure to register for program BEFORE completing this information form
**This form must be completed by a parent or legal guardian of a minor child (“participant”), or can be completed by themselves if “participant” is 18 years of age or older.
** The Equestrian Physical Evaluation Form (section 8) must be printed, completed and signed by a physician
** Leave 10-15 minutes to complete this form; must be done in one session
** One form per participant
** Participants may not attend any programs without all forms completed and returned
