The school will not give your child their prescribed medicine unless you complete this form.
Medicines MUST be in the original container as dispensed by the pharmacy.
If more than one prescribed medicine is to be given, a separate form should be completed for each prescription.
Short-term medicines need to be handed to reception staff at the start of each day and collected at the end.
Long-term repeat medications and those which are considered 'controlled' need to be discussed with Mrs Glover in Learning Support.
