MIHS Boys Soccer Mother's Day Fundraiser - Flowers Order Form

Each order form can be used for deliveries to up to 5 recipients. If ordering for delivery to more than 5 recipients, please submit multiple order forms. Thank you!

ORDER DEADLINE: 5pm on 5/1

DELIVERY DATE: 5/4 (deliveries only to Mercer Island addresses)

PAYMENT INFORMATION: $35 per bouquet. Pay via Zelle to (650) 430-1007 or Venmo to @teamchou5 (last 4 digits of phone number for verification purposes is 1007). If you cannot pay via Zelle or Venmo, you can pay via check mailed or delivered to Kevin Chou at MIBSBC, PO Box 936, Mercer Island, WA 98040 (please make the checks payable to "Kevin Chou").

If you send payment via Zelle or Venmo, please treat it as sending a payment to a friend (DO NOT mark it as a purchase of product), so that Zelle and Venmo do not charge us a transaction fee.

DELIVERY CONFIRMATION: With each delivery, we will text you (the purchaser, not the recipient) a picture of the bouquet left at the doorstep of the recipient. There will not be a note on the bouquet so it will be up to you to notify the recipients.
