This survey is for an article that will shares how various fellowships and training grants are being reported on tax forms (or not) for 2021.

This survey is for US citizens, permanent residents, and residents for tax purposes who are US-based graduate students, postdocs, and postbacs.

PhD trainees are confused by the tax forms they receive (or don't) and search for help. I want to point them to high-quality resources. Your anonymous input is vital for this article; thank you in advance for filling out this survey. If you received two forms from independent funding sources, please fill out the form twice.
