The following survey will provide us with all the information we need to sign you/your son up for the 2023 Lava Bear Ultimate team. We will be asking for contact info, birthdate, and address; in addition to your tourney availability this year. There is a release waiver at the end of the survey that must be signed by someone 18 years of age or older. Please coordinate between players and parents to make sure you have everything you need to complete the survey.
