If your child is attending BCA Saturday after school dance class, UV Performing Arts staff will pick your child up after BCA Saturday class and escort to the dance room. Pick up details are below.

BCA土曜学校放課後クラブへお申込みの方は、学校終了後、UV Performing Arts スタッフがダンスルームまで誘導いたします。お迎えは正面玄関となります。時間は下記をご確認ください。UV Performing Artsより、1-2日で連絡が無い場合は、お手数ですが再度お申込をお願いします。

The undersigned gives permission to UV Performing Arts and its’ owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. In case of emergency or injury while participating, I authorize medical care for my child and accept responsibility for medical expenses’ request our doctor/physician. /クラス中に、怪我または救急事項が発生した際、担当の講師・責任者に、医療機関への連絡または緊急手当てを任せる事に同意します。その際発生した治療費の責任を取る事へ同意します。

I understand that participation in these classes and activities shall be at my sole risk. I am fully aware of the risk involved in these activities and that unanticipated danger may arise. I waive all claims for damage, loss, or theft to my property arising out of or in connection with use of this facility and the surrounding premises or for injury including death, infection, or other damage to my person. I hereby hold UV Performing Arts and BCA and its owners and teachers harmless from all claims which may be brought against them by the student or on student’s behalf for any such injuries or claims./ 建物内外の清掃、換気、消毒、安全を第一としていますが、UV Performing Arts または建物内、駐車場にて、クラス内での怪我、死に至る事故、コロナウィルス等感染などが発生した場合、自己責任を認め、UV Performing Arts及びBCA Saturday School の経営者、責任者への訴訟を起こさない事に同意します。

I agree that no cell phone use during class time and no video taping without instructor's permission./クラス中の写真、動画撮影は、講師の許可が無い場合禁止されています。

We accept Zelle (no fee), credit card, cash, or check. Transaction fee may occurs for credit card payment./お支払い方法は、Zelle, Credit card, cash, check にて受付しております。クレジットカードご利用の際は手数料が発生します。

Payment must be submitted before your dance lesson. ／お支払いは、受講クラス前にお願いします。

Before and After Class: UV Performing Arts is not responsible for providing care to students before or after class. If student is left at the studio unattended for over 10 minutes, a fee of $1 per minute after class ending time will be charged to your account.／ダンスクラス終了後、10分までは講師がお子様の管理を致します。10分以降より、＄1のチャイルドケアー料金が発生されます。

Up until the second class of the session, if need for cancellation arises, class credit will be issued for the tuition paid deducting any classes taken. There is no class credit issued beyond the second class. There are no refunds on tuition, missed classes or class cards./ご都合により、回数券をキャンセルされる場合は、2回目のクラスまで受付ております。その際、受講されたクラス分を引いて返金させて頂きます。3回目以降のキャンセルによる返金は受け付けておりませんのでご了承ください。

Class Attire: Open Style - Comfortable cloth no skirt, not jeans, (no outside shoes, please) and gym sneakers or jazz shoes /クラス着:ジーンズ、スカート以外の動きやすい服装。外履禁止。JazzシューズまたはGymシューズ

No chewing gum and eating food are allowed during dance class／レッスン中の飲食やガムは禁止されています。軽いスナックは可能です。

I will let my child to bring some healthy snack and water bottle 休憩時用のヘルシースナックと飲み物（水または麦茶）を用意します。

I agree that registering UV Performing Arts membership sets to receive messages for appointment reminders and information about your class include news letter. /メンバーシップ登録により、クラス情報や変更等のお知らせを受け取ることに同意します。

As my child is attending BCA Saturday school, I will contact UV Performing Arts (info@uvperformingarts.com), not BCA for an anciency by Thursday. BCA土曜学校からの参加者は、欠席の場合 BCAでは無くUV Performing Arts宛てへ木曜日までに連絡します。

I understand that photography and videography are subject to the laws of copyright and intellectual property. I will ask UV Performing Arts if I want to take a photo or video during the class and performance. / 振付、映像には肖像権、著作権が発生します。クラス内、公演などの写真撮影、動画撮影はUV Performing Arts の許可を得たのみ可能です。
