1) Parents (or legal guardians) must carefully and accurately complete this form for their children.
2) Parents/Guardians are responsible for providing the correct data and contact information, including email addresses and phone numbers, in order to receive important information from IHS.
3) Failure to provide the correct information will negatively impact students' achievements and success at IHS. Correct information must also be provided in the case of an emergency, God forbid.
4) Parents and Students (of age) must frequently visit the IHS website to stay informed, and up to date, on all IHS information: www.iechusaini.org/school.
5) After completing the form, the email used to complete the form will be sent a copy of the form. Please closely review the copy of your form to confirm all contact and student information is correct. If something is incorrect, please use the edit option to correct the information, or complete a new form.
