Bu formda öncelikle Yaşam Boyu Öğrenme hakkında  bilgilendirici videolar seyretmeniz ve ardından bu videolar hakkında 10 soruluk bir kuizi cevaplandırmanız gerekmektedir.

Bu kuizden en az 50 puan almanız gerekmektedir. Eğer 50'nin altında puan alırsanız forma tekrar giriş yaparak tekrar kuiz sorularını cevaplayabilirsiniz. Bitirme tezinizin notunun USİS'e girilebilmesi için bu kuizden en az 50 almanız gerektiğini lütfen unutmayınız.

Formun giriş sayfasında e-mail adresinizi doğru yazmanız önem taşımaktadır. Yanlış mail adresi yazarsanız kuiz sonucunuz size iletilemeyecektir.
